New Customer Information Form Use this form to provide us with the information needed to create a new customer record for you in our system. Without it, we cannot process your order (except counter sales). |

Sales Tax Exemption Certificate Texas law requires that we charge sales tax on all orders unless we have a state-issued Tax Exemption Certificate on file for your business*. If your corporate and/or shipping addresses are located within the state of Texas, please use appropriate form, below (also available from the Texas Comptroller’s website). If your corporate and shipping addresses are located outside the state of Texas, you can use either the appropriate form below, or the equivalent form provided by the state of your corporate address. |
